The CC form is not working as expected. Usually you ask for full name, address 1 & 2, city, state, zip, security code, I think my CC is being declined because it's not complete.

Also, no indication of what CCs are accepted. Is Amex OK? Discover? Nobody should have to guess about this. UX leaves a lot to be desired...

They use stripe to process the payments, so any CC Stripe accepts can be used: http://stripe.com/

If you're having issues the best option is to email support, there's nothing we can help you with here unfortunately.

Looking at my bank website, looks like I got charged $.10, and was declined for address mismatch... which, of course, since there's no option to put an address in. So I guess I can't get an account. Sounds like stripe is pretty deficient in handling a fairly common use case. I don't know how to get this working.

I had to contact my bank, and the payment had to be pre-authorized. What a pain in the rear.

I actually never thought the address was integral part of payment process (some sites do ask for billing info but lots also don't), but it seems like that's an actual thing that stripe supports too.

To expand a bit on terabyte's answer, you could send an email to j-novel.club's support@ in hopes that their logs have some more answers. It's better than forums for handling PI or if you want a prompt response. You can probably still send one requesting that feature or mention @sam-pinansky here repeatedly.

Taking any information beyond the card number, its expiration date, and the security code on the back is optional.
It can be used in the verification process but it doesn't need to be. Basically, taking more info makes it more annoying for the user (have to type more info in, give up more personal information), but helps reduce credit card fraud for us, J-Novel Club. Every bank/credit card issuing company can have their own standards and systems about which charges to approve/disapprove. However this is the first time I've heard of one that actually required an address?
I have heard of ones that will block any internet transactions by default for brand new cards or cards issued to students, etc.

So in any case, we don't take that extra info because we don't need it to sell you what we are offering, and in 99% of cases it doesn't affect anyone's credit card processing. I'm not sure about the specific card you are attempting to use, but that's really up to the policies of your bank.
